Download the approved 2027 form for your child's level. Complete it clearly, attach the required documents, and submit it with proof of payment for the application fee.

For learners entering Pre-Primary. Covers personal particulars, parents/guardians, required documents, and terms & agreements.
For learners entering Grades 1 to 3. Includes sections for both parents/guardians, health information, and the consent declaration.
For learners entering Grades 4 to 7. Includes sections for both parents/guardians, health information, and the consent declaration.
Required for learners who will stay in the hostel. Includes parent/guardian details and undertaking to abide by hostel rules and fee payments.
Required Documents: Child's birth certificate or baptism certificate, two passport photographs, previous grade report (except where not applicable to Pre-Primary), and proof of payment of the application fee ().
